Antecedentes: La investigación sobre fluidoterapia y monitorización hemodinámica perioperatorias es difícil y costosa. Los objetivos del presente estudio fueron resumir y priorizar estas cuestiones, en orden de importancia investigadora. Métodos: Cuestionario estructurado electrónico Delphi a lo largo de tres rondas entre 30 expertos en fluidoterapia y monitorización hemodinámica identificados a través del Subcomité de Fluidoterapia y Monitorización hemodinámica de la Sección de Hemostasia, Medicina Transfusional y Fluidoterapia de la Sociedad Española de Anestesiología y Reanimación. Resultados: Se identificaron 77 cuestiones, que se clasificaron en orden de priorización. Las cuestiones se categorizaron en temas de cristaloides, coloides, monitorización hemodinámica y otros. Se categorizaron 31 cuestiones como de prioridad investigadora esencial, para determinar si los algoritmos de optimización hemodinámica intraoperatorios basados en el índice de predicción de hipotensión invasivo o no invasivo frente a otras estrategias de manejo podrían reducir la incidencia de complicaciones postoperatorias, así como si el uso de biomarcadores del estrés renal junto con un protocolo de fluidoterapia dirigido por objetivos podría reducir la estancia hospitalaria y la incidencia de insuficiencia renal aguda en pacientes adultos sometidos a cirugía no cardiaca, lográndose el más alto consenso. Conclusiones: El Subcomité de Fluidoterapia y Monitorización hemodinámica de la Sección de Hemostasia, Medicina Transfusional y Fluidoterapia de la Sociedad Española de Anestesiología y Reanimación utilizará estos resultados para la realización de investigación.(AU)

BACKGROUND: Research in fluid therapy and perioperative hemodynamic monitoring is difficult and expensive. The objectives of this study were to summarize these topics and to prioritize these topics in order of research importance. METHODS: Electronic structured Delphi questionnaire over three rounds among 30 experts in fluid therapy and hemodynamic monitoring identified through the Fluid Therapy and Hemodynamic Monitoring Subcommittee of the Hemostasis, Transfusion Medicine and Fluid Therapy Section of the Spanish Society of Anesthesiology and Critical Care. RESULTS: 77 topics were identified and ranked in order of prioritization. Topics were categorized into themes of crystalloids, colloids, hemodynamic monitoring and others. 31 topics were ranked as essential research priority. To determine whether intraoperative hemodynamic optimization algorithms based on the invasive or noninvasive Hypotension Prediction Index versus other management strategies could decrease the incidence of postoperative complications. As well as whether the use of renal stress biomarkers together with a goal-directed fluid therapy protocol could reduce hospital stay and the incidence of acute kidney injury in adult patients undergoing non-cardiac surgery, reached the highest consensus. CONCLUSIONS: The Fluid Therapy and Hemodynamic Monitoring Subcommittee of the Hemostasis, Transfusion Medicine and Fluid Therapy Section of the Spanish Society of Anesthesiology and Critical Care will use these results to carry out the research.

OBJECTIVES: To conduct brainstem MRI shape analysis across neurodegenerative parkinsonisms and control subjects (CS), along with its association with clinical and cerebrospinal fluid (CSF) correlates. METHODOLOGY: We collected demographic and clinical variables, performed planimetric and shape MRI analyses, and determined CSF neurofilament-light chain (NfL) levels in 84 participants: 11 CS, 12 with Parkinson's disease (PD), 26 with multiple system atrophy (MSA), 21 with progressive supranuclear palsy (PSP), and 14 with corticobasal degeneration (CBD). RESULTS: MSA featured the most extensive and significant brainstem shape narrowing (that is, atrophy), mostly in the pons. CBD presented local atrophy in several small areas in the pons and midbrain compared to PD and CS. PSP presented local atrophy in small areas in the posterior and upper midbrain as well as the rostral pons compared to MSA. Our findings of planimetric MRI measurements and CSF NfL levels replicated those from previous literature. Brainstem shape atrophy correlated with worse motor state in all parkinsonisms and with higher NfL levels in MSA, PSP, and PD. CONCLUSION: Atypical parkinsonisms present different brainstem shape patterns which correlate with clinical severity and neuronal degeneration. In MSA, shape analysis could be further explored as a potential diagnostic biomarker. By contrast, shape analysis appears to have a rather limited discriminant value in PSP. El Patient Blood Management (PBM) es un enfoque centrado en el paciente, sistemático y basado en la evidencia, destinado a mejorar los resultados de las pacientes mediante el manejo y la preservación de la propia sangre. Consta de tres pilares clásicos que se llevan a cabo de forma multidisciplinar y multimodal: optimización de la masa eritrocitaria, reducción de la hemorragia y mejoría de la tolerancia a la anemia. La optimización de la masa eritrocitaria incluye el diagnóstico precoz de la anemia y su tratamiento con hierro (oral o intravenoso), siendo el déficit de hierro la causa más frecuente de anemia en el contexto de sangrado uterino anómalo (SUA). La reducción de la hemorragia incluye estrategias destinadas a tratar la causa del SUA, sea de forma quirúrgica, médica o ambas. La mejoría en la tolerancia a la anemia incluye el umbral de transfusión restrictivo en pacientes sin hemorragia activa, administrando el mínimo número de unidades de hematíes para paliar los síntomas (Single Unit Transfusion). En el presente texto se realiza una revisión de la evolución y de la situación actual del PBM en sus tres pilares, centrándose en los aspectos concretos más directamente aplicables al sangrado uterino anómalo.(AU)


Patient Blood Management (PBM) is a systematic, evidence-based, patient-centred approach aimed at improving patient outcomes through the management and preservation of their own blood. It consists of three classic pillars performed in a multidisciplinary and multimodal way: optimization of erythrocyte mass, reduction of haemorrhage, and improvement of tolerance to anaemia. Optimization of erythrocyte mass includes the early diagnosis of anaemia and its treatment with iron (oral or intravenous), with iron deficiency being the most frequent cause of anaemia in the context of abnormal uterine bleeding (AUB). Bleeding reduction includes strategies aimed at treating the cause of AUB either surgically, medically, or both. The improvement in tolerance to anaemia includes the restrictive transfusion threshold in patients without active bleeding, administering the minimum number of red blood cell units to alleviate symptoms (single unit transfusion). This text reviews the evolution and current situation of the three pillars of PBM, focusing on the specific aspects most directly applicable to abnormal uterine bleeding.(AU)

Genicular nerves block is a promising technique to treat acute postoperative pain in total knee arthroplasty. Similar to surgeon-administered local infiltration analgesia, it targets sensory branches from the knee capsule, but through a selective ultrasound-guided injection that reduces local anaesthetic dose (150 ml ropivacaine 0.2% with local infiltration analgesia vs. 20 ml with genicular nerves block). This randomised non-inferiority trial compared the analgesic efficacy of genicular nerves block vs. local infiltration analgesia in the first 24 h following total knee arthroplasty. Sixty patients were randomly allocated to receive either ultrasound-guided block of five genicular nerves or local infiltration analgesia. The primary outcome was rest pain numeric rating scale (0-10) at 24 h. Secondary outcomes included pain numeric rating scale (rest and movement) and cumulative opioid consumption during the first 24 h. We analysed 29 patients in the genicular nerves block group and 30 in the local infiltration analgesia group. We found that the median difference (95%CI) in postoperative rest pain at 24 h (non-inferiority criteria, Δ = 1) was -1.0 (-2.0 to 1.0, p < 0.001). Median difference in cumulative opioid consumption was 0.0 mg (-3.0-5.0, p < 0.001) meeting the non-inferiority criteria, Δ = 23 mg. We conclude that genicular nerves block of five nerves provides non-inferior analgesia in the first 24 h following surgery compared with local infiltration analgesia, but with a considerable reduction in the local anaesthetic dose.

INTRODUCTION: Differential diagnosis between Parkinson's disease (PD) and atypical parkinsonisms (APs: multiple system atrophy[MSA], progressive supranuclear palsy[PSP], corticobasal degeneration[CBD]) remains challenging. Lately, cerebrospinal fluid (CSF) studies of neurofilament light-chain (NFL) and RT-QuIC of alpha-synuclein (α-SYN) have shown promise, but data on their combination with MRI measures is lacking. OBJECTIVE: (1) to assess the combined diagnostic ability of CSF RT-QuIC α-SYN, CSF NFL and midbrain/pons MRI planimetry in degenerative parkinsonisms; (2) to evaluate if biomarker-signatures relate to clinical diagnoses and whether or not unexpected findings can guide diagnostic revision. METHODS: We collected demographic and clinical data and set up α-SYN RT-QuIC at our lab in a cross-sectional cohort of 112 participants: 19 control subjects (CSs), 20PD, 37MSA, 23PSP, and 13CBD cases. We also determined CSF NFL by ELISA and, in 74 participants (10CSs, 9PD, 26MSA, 19PSP, 10CBD), automatized planimetric midbrain/pons areas from 3T-MRI. RESULTS: Sensitivity of α-SYN RT-QuIC for PD was 75% increasing to 81% after revisiting clinical diagnoses with aid of biomarkers. Sensitivity for MSA was 12% but decreased to 9% with diagnostic revision. Specificities were 100% against CSs, and 89% against tauopathies raising to 91% with diagnostic revision. CSF NFL was significantly higher in APs. The combination of biomarkers yielded high diagnostic accuracy (PD vs. non-PD AUC = 0.983; MSA vs. non-MSA AUC = 0.933; tauopathies vs. non-tauopathies AUC = 0.924). Biomarkers-signatures fitted in most cases with clinical classification. CONCLUSIONS: The combination of CSF NFL, CSF RT-QuIC α-SYN and midbrain/pons MRI measures showed high discriminant ability across all groups. Results opposite to expected can assist diagnostic reclassification.

ANTECEDENTES: La implementación de los programas Patient Blood Management (PBM) es variable en Europa, incluso en centros en los que estos programas están bien establecidos, donde existe variabilidad en cuanto a prácticas transfusionales. OBJETIVOS Y MÉTODOS: Realizamos una encuesta para valorar la práctica actual sobre PBM perioperatoria en pacientes programados para artroplastia total de cadera y rodilla, entre los investigadores involucrados en el Estudio POWER.2 en España (estudio observacional prospectivo que evaluaba las vías de recuperación intensificada en cirugía ortopédica). RESULTADOS: Se obtuvo un total de 322 respuestas (37,8%). El 50% de los respondedores revisaban los niveles de hemoglobina, al menos 4 semanas antes de la cirugía; el 35% trataba a todos los pacientes anémicos, aunque el 99,7% consideraba que la detección y tratamiento de la anemia preoperatoria podrían influir en los resultados postoperatorios. La falta de infraestructuras (76%) y la falta de tiempo (51%) fueron los principales motivos para no tratar a los pacientes anémicos. El estatus del hierro es revisado antes de la cirugía por el 19% de manera rutinaria, y el 36% lo evalúa únicamente en pacientes anémicos. Hb<9,9g/dl es el valor umbral para demorar la cirugía para el 61% de los clínicos, y el 22% consideraría transfundir preoperatoriamente a los pacientes clínicamente estables sin sangrado activo. El valor umbral para transfundir a los pacientes sin enfermedad cardiovascular es 8g/dl para el 43% y 7g/dl para el 34% de los respondedores; el 75% de los facultativos considera que utiliza «umbrales restrictivos», y el 90% sigue la política transfusional uno a uno (single unit). CONCLUSIONES: Los resultados de nuestra encuesta muestran la variabilidad en la práctica clínica en PBM en cirugía ortopédica mayor, a pesar de ser el tipo de cirugía con más tradición en estos programas

The threshold to transfuse patients without cardiovascular disease is 8 g/dl for 43%, and 7 g/dl for 34% of the responders; 75% of clinicians consider they use "restrictive thresholds", and 90% follow the single unit transfusion policy. CONCLUSIONS: The results of our survey show variability in clinical practice in Patient Blood Management in major orthopaedic surgery, despite being the surgery with the greatest tradition in these programs

Background: Perioperative tranexamic acid (TXA) use can reduce bleeding and transfusion requirements in several types of surgery, but level I evidence proving its effectiveness in major spine surgery is lacking. This study was designed to investigate the hypothesis that TXA reduces perioperative blood loss and transfusion requirements in patients undergoing major spine procedures. Methods: We conducted a multicentre, prospective, randomized double-blind clinical trial, comparing TXA with placebo in posterior instrumented spine surgery. Efficacy was determined based on the total number of blood units transfused and the perioperative blood loss. Other variables such as the characteristics of surgery, length of hospital stay, and complications were also analysed. Results: Ninety-five patients undergoing posterior instrumented spine surgery (fusion of >3 segments) were enrolled and randomized: 44 received TXA (TXA group) and 51 received placebo (controls). The groups were comparable for duration of surgery, number of levels fused, and length of hospitalization. Transfusion was not required in 48% of subjects receiving TXA compared with 33% of controls (P = 0.05). Mean number of blood units transfused was 0.85 in the TXA group and 1.42 with placebo (P = 0.06). TXA resulted in a significant decrease in intraoperative bleeding (P = 0.01) and total bleeding (P = 0.01) relative to placebo. The incidence of adverse events was similar in the two groups. Conclusions: TXA did not significantly reduce transfusion requirements, but significantly reduced perioperative blood loss in adults undergoing major spinal surgery. Objetivos. La artroplastia total de rodilla (ATR) presenta una elevada tasa transfusional. En nuestro protocolo, indicamos el empleo del recuperador de drenajes postoperatorios (RDPO) en los pacientes con contraindicación al ácido tranexámico. Analizamos el efecto de los RDPO respecto la tasa transfusional y sus costes en pacientes intervenidos de ATR. Material y métodos. Se incluyeron prospectivamente 518 pacientes. De ellos, 434 recibieron ácido tranexámico y en 84 se contraindicó. Calculamos la masa eritrocitaria, el volumen sanguíneo y el porcentaje de volumen sanguíneo perdido. Controlamos las incidencias asociadas al uso del recuperador y la tasa de transfusión. Realizamos un análisis de costes asociados a los métodos de prevención de transfusión alogénica. Resultados. En 10 de los 84 pacientes no candidatos a ácido tranexámico no colocamos el RDPO. En los 74 en los que colocamos un RDPO, reinfundimos 158 ± 72 ml de masa eritrocitaria. La tasa de transfusión alogénica fue del 36%, y en los que no se colocó, del 52%. Con el empleo de RDPO el riesgo relativo de transfusión se situó en el 0,69 (0,41-1,16), con una reducción de riesgo absoluto del 16% (-8,-40%). El número de pacientes a tratar para evitar una transfusión alogénica fue de 7. Los costes tangibles directos para evitar una transfusión alogénica se situó en 1.610€. No observamos complicaciones asociadas a la reinfusión de la sangre recuperada. Conclusiones. El empleo de RDPO tras ATR precisa de su empleo en 7 pacientes para evitar una transfusión alogénica, con un coste más de 10 veces superior al del concentrado alogénico


Objectives. Total knee arthroplasty (TKA) has a high transfusion rate. In our protocol, the use of postoperative cell salvage is indicated in patients with contraindications to tranexamic acid (TA). An analysis was performed on the effect of post-operative cell salvage (POCS) regarding transfusion rate and costs in patients undergoing TKA. Material and methods. A prospective analysis was conducted on 518 patients, of whom 434 received TA, and 84 were contraindicated. The red cell mass, blood volume, and the percentage of lost blood volume were calculated. Incidents associated with the use of post-operative re-perfusion of drained blood and the rate of transfusion were recorded. An analysis was performed on the costs associated with allogeneic transfusion prevention methods. Results. A POCS drain was not inserted in 10 out of the 84 patients not candidates for TA. In the 74 in which it was placed, 158 ± 72 ml of red cell mass was reinfused. The allogeneic transfusion rate was 36%, and was 52% in those with no drain inserted. Relative risk of transfusion using POCS was 0.69 (0.41 to 1.16) with an absolute risk reduction of 16% (-8 to 40%). The number needed to treat to avoid allogeneic transfusion was 7. The direct costs to avoid allogeneic transfusion were €1,610. No complications associated with blood re-infusion were observed. Conclusions. The use of POCS would be required in 7 patients after TKA to avoid one allogeneic transfusion with a cost over 10 times that of a transfusion of red cell concentrates

Esta Guía de Práctica Clínica responde a preguntas clínicas sobre seguridad en la elección de fluido (cristaloide, coloide o Hidroxietilalmidón 130) en pacientes que precisan restauración volémica en el periodo perioperatorio de cirugía no cardiaca. A partir del resumen de la evidencia, se elaboraron las recomendaciones siguiendo la metodología GRADE. En esta población se sugiere la fluidoterapia basada en la administración de cristaloides, (recomendación débil, calidad de la evidencia baja). En las situaciones en las que la restauración volémica no se alcance sólo con cristaloides, se sugiere utilizar coloides sintéticos (Hidroxietilalmidón 130 o gelatina fluida modificada) en lugar de Albúmina 5% (recomendación débil, calidad de la evidencia baja). La elección y dosificación de coloide deberán basarse en las características del producto, comorbilidad del paciente y experiencia del anestesiólogo (AU)

Objetivo: Evaluar la efectividad del tratamiento con hierro intravenoso, solo o asociado a eritropoyetina (rHuEPO), mediante el incremento de hemoglobina (Hb). Como objetivo secundario se analizó la relación del incremento de Hb con los parámetros utilizados para evaluar la anemia. Material y método: Estudio observacional retrospectivo realizado en 2 centros entre enero de 2005 y diciembre de 2009. Se incluyeron pacientes sometidos a cirugía ortopédica electiva con déficit de hierro y que fueron tratados con hierro sacarosa intravenosos solo o asociado a rHuEPO. Se analizó la efectividad del tratamiento mediante la diferencia entre la Hb basal y la Hb previa a la cirugía y se valoró la relación entre los parámetros hematimétricos y del metabolismo del hierro con la efectividad del tratamiento. Resultados: Se incluyeron 412 pacientes que recibieron una mediana de 800 mg de hierro sacarosa. A 125 pacientes (30,4%) se les asoció 2,4 viales de rHuEPO. El incremento de Hb fue de 0,8 (1,1) g/dL en los pacientes tratados con hierro intravenoso y de 1,5 (1,3) g/dL en los que se asoció rHuEPO (p < 0,01). El incremento de Hb en los pacientes tratados con hierro se correlacionó con el porcentaje de hematíes hipocromos (r = 0,52) y el valor del receptor soluble de la transferrina (r = 0,59). Conclusiones: La efectividad del tratamiento con hierro sacarosa en pacientes con déficit de hierro para la optimización de la Hb preoperatoria fue moderada, siendo mayor con la administración adyuvante de eritropoyetina. La determinación de los parámetros funcionales del estado del hierro nos podría guiar para mejorar la efectividad del tratamiento (AU)

Objetivo: El estudio FILTRO pretende conocer las medidas profilácticas del sangrado quirúrgico excesivo que se aplican en España y su impacto en la evolución del paciente. Material y métodos: Estudio epidemiológico retrospectivo en 1.180 pacientes intervenidos de artroplastia total primaria de cadera o rodilla entre enero y septiembre de 2010. El estudio fue aprobado por el CEIC del Hospital Clínic de Barcelona. Antes de la cirugía se administró hierro y/o eritopoyetina en un 4,1% de pacientes. Durante la cirugía, un 2,8% fue tratado con fármacos antifibrinolíticos, en un 11,8% se empleó recuperador de sangre intraoperatorio y en el 12,7% se aplicó la autotransfusión. Resultados: El valor medio de Hb en el preoperatorio fue de 13,5 g/dL, y antes del alta, de 10,6g/dL. Un 33,2% de los pacientes recibieron transfusión durante el ingreso y se asoció a la utilización de drenaje (p=0,0033) y a la autotransfusión (p=0,0002). El uso de antifibrinolíticos disminuyó la necesidad de transfusión (p=0,0025). Se registraron complicaciones tras la cirugía en el 5,5% de la muestra, sin observarse asociación con las técnicas de ahorro de sangre empleadas. Conclusión: A pesar de disponer en nuestros hospitales de procedimientos y medidas eficaces para reducir la necesidad de transfusiones de sangre alogénica, no parece que estas se apliquen de forma generalizada en la práctica clínica de los hospitales españoles (AU)


Objective: The aim of the FILTRO study was to determine which were prophylactic measures of excessive surgical bleeding that are being implemented in Spain in patients undergoing elective total primary arthroplasty of the hip or knee, and the impact of these measures on the patient outcomes in short term. Material and methods: We included 1,180 patients, underwent surgery for primary hip and knee replacement. All data were collected retrospectively. Socio-demographic and clinical data were recorded from the clinical history, including techniques and drugs administered, as well as the patient outcomes during admission and after discharge consultation. The blood-saving measures used were: before surgery, iron or erythropoietin in 4.1% of the sample; during surgery, antifibrinolytic drugs in 2.8%, a blood-recovery system in 11.8% and 12.7% used autotransfusion. 33.2% of patients received blood transfusion during hospitalization. Results: The mean value of Hb before surgery was 13.5 g/dL and before discharge was 10.6 g/dL. The need for transfusion was higher in patients using drainage (p=0.0033) and those who opted for autotransfusion (p=0.0002). The use of antifibrinolytic drugs decreased the transfusion requirements (p=0.0025). Postoperative complications were recorded in 5.5% of the sample: deep venous thrombosis in 0.7% of cases, bleeding in 1.9% and infection in 2.8%, and there was no association between these and the use or not either blood-saving method. Conclusion: Although Spanish hospitals have procedures and measures to reduce the need for allogeneic blood transfusions, it seems they are still not generally applied in clinical practice (AU)
